Telstra CEO Vicki Brady has told an Australian Senate inquiry that last week’s nationwide telecommunications outage was likely triggered by an undocumented network design change combined with a missed software update, exposing weaknesses in operational change management and network maintenance procedures.
The disruption affected thousands of customers across Australia, interrupting mobile and voice services, electronic payment systems, rail operations, and emergency communications, including calls to the country’s Triple Zero emergency service.
According to Brady, the incident began during routine maintenance on network timing and synchronization equipment. Restarting the device unexpectedly reset its internal date to 2006, causing authentication certificates across the network to become invalid and triggering widespread service failures.
The investigation found that the equipment behaved differently because engineers had previously implemented a design modification intended to solve an earlier technical issue. However, that change was never properly documented, meaning maintenance personnel were unaware of its impact during the restart process.
Compounding the problem, a recommended software update had not been installed on the affected device. Brady acknowledged that either proper documentation or timely software maintenance could likely have prevented the outage.
The company has launched an internal review to determine why the design modification was not formally recorded, why the software update remained outstanding, and how risk management processes can be strengthened to prevent similar failures in the future.
The outage has renewed scrutiny of Australia’s telecommunications infrastructure following several major incidents affecting the sector in recent years. Previous disruptions have included a prolonged Optus outage, a large-scale cybersecurity breach exposing customer data, and failures that affected emergency communication services.
These repeated incidents have prompted regulators and lawmakers to place greater emphasis on network resilience, operational governance, and infrastructure reliability, particularly as telecommunications systems support increasingly critical services across finance, transportation, healthcare, and emergency response.
Industry experts note that while cybersecurity often dominates discussions around digital infrastructure, configuration management, software maintenance, and internal operational controls remain equally important components of network reliability.
The Telstra incident serves as a reminder that seemingly routine maintenance can have system-wide consequences when documentation, change management, and software lifecycle processes are not rigorously maintained.
